How does the support with PROXYMO unfold?

Every situation is different, but our method rests on a simple logic: understanding, analysing, guiding, coordinating and ensuring continuity when necessary. PROXYMO was designed to make care pathways more readable, better prepared and more coherent between East Africa and Belgium.

Not all of the steps below apply systematically to every situation. The level of support depends on the actual need, the medical context, the care project and the expectations of the patient or family.

1. First contact and understanding of the situation.

Everything begins with an initial exchange aimed at understanding the situation, the context, the expectations, the level of urgency and the difficulties encountered. This first contact makes it possible to set a clear framework and to identify the most suitable type of support.

What PROXYMO does:

  • listens to the request;
  • rephrases the needs;
  • identifies the first useful elements;
  • provides initial guidance on the possible next steps of the journey.

What you prepare:

  • basic information about the situation;
  • immediate needs;
  • the first available documents, if you already have any.

2. File review and clarification of the need.

When necessary, we analyse the file and the available information more precisely, in order to better understand the situation, the priorities, the constraints and the options that can be considered. This step makes it possible to move away from uncertainty and to avoid misdirected steps from the outset.

What PROXYMO does:

  • structures the information received;
  • identifies the points to be clarified;
  • prepares the orientation towards the relevant interlocutors.

What you prepare:

  • medical reports;
  • examinations, imaging, prescriptions or relevant documents;
  • anything that helps to better understand the situation.

3. Orientation towards the right interlocutors.

Once the situation is clarified, PROXYMO helps to direct the request to the professionals, partners or facilities most relevant to the identified need. Depending on the case, this may involve a second opinion, a teleconsultation, a consultation in Belgium or a more targeted connection with the right contacts.

What PROXYMO does:

  • helps to target the right interlocutors;
  • facilitates the understanding of each person’s role;
  • supports the overall coherence of the journey.

Expected result:

A better-targeted journey, with more coherent steps and interlocutors better suited to the situation.

4. Preparation and coordination of the relevant steps.

When several steps need to be organised, PROXYMO helps to prepare the important stages, smooth the exchanges and maintain a clear overall view of the journey. If a trip to Belgium is considered, this phase may also include the practical organisation of certain stages of the stay.

Depending on needs, this may include:

  • preparing consultations or assessments;
  • coordinating useful exchanges around the journey;
  • helping to find accommodation near the relevant facilities;
  • logistical preparation of certain stages in Belgium.

5. Support during the journey.

Some situations require more than remote coordination alone. When a particular need exists, PROXYMO can organise human support during a key phase of the journey, in order to help the patient and family better understand certain consultations, prepare their questions and move forward with greater clarity.

This support may be provided by an experienced healthcare professional, while respecting the role of each doctor and without interfering with clinical decisions.

6. Concrete support in Belgium depending on the situation.

When the situation warrants it, PROXYMO may also mobilise, through Global Care Partners, concrete services in Belgium to support the patient during their stay or after a hospitalisation.

Depending on needs and feasibility, this may include:

  • practical support around the stay;
  • appropriate nursing or care-assistant support;
  • help with continuity after a hospital stage;
  • closer coordination between the various parties involved.

7. Continuity after the main stage or after the return.

Useful coordination does not always end with a consultation, an intervention or a trip. Depending on the situation, PROXYMO helps to clarify what comes next, to maintain the continuity of exchanges and to organise follow-up after an important stage of the journey.

This may include post-hospitalisation follow-up in Belgium, coordination after the return to the country of residence, or remote exchanges when relevant. In certain strictly regulated cases, therapeutic continuity may also involve organising the sending of medication on a valid prescription and in compliance with applicable rules.

A clear method reduces uncertainty.

When roles are well understood, when steps are prepared with method and when information circulates in a more coherent way, the journey becomes more readable and less of a burden to carry for the patient and their loved ones. This is precisely what PROXYMO seeks to make possible.
